About
Limitless Horizons is a San Diego based agency dedicated to supporting individuals with intellectual and developmental disabilities as they build meaningful, self-directed lives. Vendored through the San Diego Regional Center, we deliver person-centered services that meet each individual and family where they are and help them reach for whatever comes next.
We are not a one-size-fits-all provider. We build support around the person: their goals, their family, their community, and their vision for the future. We are advocates, listeners, skilled professionals, and trusted partners committed to dignity, choice, and independence at every step.
With more than 15 years of experience serving individuals and families within California’s developmental services system, we understand that exceptional support begins with listening. Every individual has unique strengths, goals, and dreams, and we are committed to providing services that are thoughtful, responsive, and tailored to each person’s needs. Families can trust that every decision we make is guided by compassion, professionalism, and an unwavering commitment to quality.
WHAT WE OFFER
Our Services
Limitless Horizons offers four service lines, each designed to promote independence, inclusion, and personal growth. Every service is individualized, strengths based and built around the person at its center.

Coordinated Family Support (CFS)
For adults who live at home with their families, Coordinated Family Support strengthens the whole household. We work alongside individuals and their family caregivers to coordinate services, build natural and community supports, navigate complex systems, and sustain a stable, thriving living arrangement. Our goal is to help families care for the person they love while supporting that person’s growth toward greater independence.

Independent Living Services (ILS)
Independent Living Services help adults with disabilities gain the practical skills to live life on their own terms. Through individualized, hands-on training, we support individuals in mastering everyday tasks such as managing a household, budgeting and paying bills, cooking, personal care, using transportation, and accessing their community. We meet each person where they are and build the confidence and competence that make independent living possible.

Tailored Day Services (TDS)
Tailored Day Services offer a flexible, personalized alternative to traditional day programs. Rather than a fixed schedule, we design each person’s day around their unique goals, whether that means pursuing employment, continuing education, volunteering, developing new skills, or becoming more active in the community. With individualized support and a focus on real outcomes, TDS helps people build the life they envision.

Supported Living Services (SLS)
Supported Living Services make it possible for adults with disabilities to live in a home of their own with exactly the level of support they need. From a few hours a week to around the clock assistance, our services are shaped entirely by the individual’s choices and preferences. We emphasize self-determination, safety, and full community membership, helping each person live where they want, how they want, with dignity and control over their own life.

We proudly serve individuals with intellectual and developmental disabilities and their families across San Diego County. Our clients come to us through the San Diego Regional Center, and we partner closely with service coordinators, families, and community providers to ensure every person receives coordinated, compassionate, and effective support.

MEET OUR FOUNDER & CEO
Sarah Mojabi
For as long as she can remember, Sarah Mojabi has been surrounded by the people and families that Limitless Horizons was created to serve. Her journey began alongside her mother, who has devoted more than 40 years to supporting individuals with developmental disabilities and their families. Watching her mother’s unwavering compassion, dedication, and advocacy shaped Sarah’s understanding of what meaningful support truly looks like. She learned early that this work is about far more than providing services. It is about preserving dignity, building genuine relationships, and recognizing the unique strengths and potential within every individual.
Those early lessons became a lifelong calling.
From a young age, Sarah knew this was the work she was meant to do. She came to understand that meaningful support is not about doing things for people. It is about standing beside them, celebrating their strengths, and helping them build the lives they envision for themselves.
For more than 15 years, Sarah has had the privilege of supporting individuals and families served through the Regional Center system. Throughout her career, she has celebrated milestones, helped navigate life’s challenges, and built lasting relationships grounded in trust, compassion, and respect. These experiences deepened her commitment to creating an agency where every person feels seen, heard, and valued.
That vision became Limitless Horizons.
Sarah founded Limitless Horizons on the belief that every individual deserves personalized support that empowers them to live a life of purpose, independence, and fulfillment. She believes the most meaningful services begin with listening, understanding each person’s goals, honoring their choices, and providing individualized support that helps them thrive.
As a proud Iranian American, Sarah was raised in a culture that values family, hospitality, compassion, and caring for one another. Those values continue to shape the heart of Limitless Horizons. Every individual and family who walks through the agency’s doors is welcomed with kindness, respect, and a genuine commitment to helping them achieve their goals.
